A Practitioner's Guide to Choosing a Fish Oil

Reviewed by Carolyn Mulally BS, RN, HN-BC · Holistic Health Partners

Fish oil is one of the most-researched supplements there is — and one of the easiest to buy badly. The products above are the omega-3 formulas our clinic actually recommends: professional-grade fish oils from Metagenics, Thorne, Pure Encapsulations, Designs for Health, Xymogen, and Ortho Molecular, selected for potency, purity, and form.

Why professional-grade fish oil is different

Retail fish oil often delivers a small amount of EPA and DHA per softgel, padded with filler oils, and may sit in a warehouse long enough to oxidize. Professional-grade formulas deliver clinically meaningful omega-3 doses per serving, use triglyceride or monoglyceride forms your body absorbs more readily, and are third-party tested for purity, heavy metals, and freshness. You take fewer softgels and actually get what the label promises.

EPA, DHA, full-spectrum — or SPM?

EPA-leaning formulas are typically chosen to support cardiovascular health and a healthy inflammatory response. DHA is the omega-3 most concentrated in the brain and eyes, so DHA-forward products are often used for cognitive and visual support. Full-spectrum formulas cover both bases for everyday use. You’ll also find SPM products here — specialized pro-resolving mediators, a newer class of omega-3 derivatives that support the body’s natural resolution processes.

How our clinic selects these products

Every product in this category was chosen by our clinical team the same way we’d choose for our own patients: meaningful EPA/DHA per softgel, absorbable forms, verified third-party testing, and manufacturers with genuine practitioner-channel quality standards. If a fish oil doesn’t meet that bar, it isn’t here.

What's the difference between EPA and DHA?

Both are omega-3 fatty acids. EPA is generally emphasized for cardiovascular support and maintaining a healthy inflammatory response, while DHA is the dominant omega-3 in brain and eye tissue, making it the focus for cognitive and visual support. Many people do well with a full-spectrum formula that provides both.

What are SPM fish oil products?

SPMs — specialized pro-resolving mediators — are compounds derived from omega-3 fatty acids that support the resolution phase of the body's natural inflammatory response. They're a newer, more targeted option often used alongside or after standard fish oil.

How can I tell a fish oil is high quality?

Look for the triglyceride or monoglyceride form rather than ethyl esters, third-party testing for purity and oxidation, and a meaningful amount of EPA and DHA per softgel — not just total "fish oil" milligrams. Every product our clinic carries meets these standards.

Can I take fish oil with my medications?

If you take blood-thinning medication or are preparing for surgery, talk with a practitioner before starting fish oil. Our product consultations are free, and we'll give you a straight answer — including when to check with your doctor first.
